All copays are due prior to seeing the provider. If you are unable to pay your copay, unfortunately your appointment will be rescheduled.
If you have a past due patient balance, you will be asked to pay it. You may also pay your bill online before your visit.
We ask at every visit to update to your insurance information. This is to ensure that your claims are billed properly, so that the claim is not adjusted to your responsibility. Please make sure you bring your insurance card to each visit.
All HMO Patients are responsible for getting their referrals/authorizations from their primary care physician. If you do not have your referral at the time of your appointment, your appointment will be rescheduled.
Any insurance deductibles unmet at the time a surgery is scheduled will be required to be paid prior to surgery. Arrangements can be made with a Financial Counselor to set-up payment(s) of your deductible.
